Make an Appointment Initial Appointment For your initial appointment at Carle Spine Institute, please arrive 15 minutes before your scheduled appointment. This will allow us sufficient time to complete all of the necessary paperwork. Referrals Some health plans have specific rules you must follow in order to be seen by a specialist. If your health insurance company requires a referral from a primary care physician, remember that it is YOUR responsibility to provide us with the necessary referral information and documents. You must have this referral information with you during your initial appointment to be seen by the doctor. Appointment Request Form Cancellations As a regional center of excellence, there can be a several-day wait to see one of our physicians. Consequently, if you have to cancel an appointment, please call us as soon as possible, preferably at least 24 hours before your scheduled appointment. This enables another patient to have your time slot.

contract with insurance providers separately and may or may not choose
to participate in all of the same insurance plans. Patients are urged
